Fredericksburg Health Inspections: Dead Roaches, Moldy Strawberries


FREDERICKSBURG, VA — Several facilities around the Fredericksburg area recorded violations during health department inspections between Dec. 14 and Dec. 20.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important “priority” violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Skrimp Shack, 736 Warrenton Rd Suite #105 Fredericksburg, VA 22406
    • Date inspected: Dec. 14
    • Total violations: 7
      • Observed a food employee handle personal clothing then engaged in food preparation without first washing their hands. Employee was instructed to wash their hands.
      • The concentration of the quaternary ammonium sanitizer solution at the 3 compartment sink and the sanitizer bucket measured 0 PPM. Sanitizer solution was remade (300 PPM).
      • Observed hush puppies mix (49’F) stored at improper temperature in the prep unit (38’F) near the wired shelving unit. Hush puppies dry mix contains milk and eggs. item was placed in the unit overnight. Person in charge (PIC) stated that the item is usually stored at room temperature. Item was discarded.
      • Observed a bottle of disinfectant spray stored next to single serve articles in on the shelving unit by the cash register. The person in charge relocated the bottle.
      • A splash guard is needed between the hand washing sink and the prep table by the breading station to prevent contamination.
      • The nonfood contact surfaces of the interior of all reach in coolers and shelves are noted with an accumulation of grime and debris.
      • Observed floor tiles under equipment and walls behind the equipment are in need on detailed cleaning.
  • McDonald’s (Stanstead) #5123, 44 Stanstead Road Fredericksburg, VA 22405
    • Date inspected: Dec. 14
    • Total violations: 9
      • Observed the handwashing sings were missing from the men and womens bathrooms. During the inspection EHS provided handwashing signs.
      • Observed the two microwaves of the facility were observed soiled.
      • Observed one pan of lettuce, which time as a public health control is being used, with the wrong date marking sticker. Sticker was marked for a 24 hour used by time instead of the required 4 hour when using time a public health control.
      • Observed food employees working without hair restraints.
      • Observed food employees working while wearing wrist jewelry.
      • Observed several rolls of paper towels stored on a shelf unprotected from contaminations. EHS spoke with the person in charge about keeping the rolls of paper towels in the box they were received in to ensure paper towels are protected from contamination.
      • Observed the following: 1. wire racks in the dish washing area holding equipment that are rusting 2. ice build up on the walls, floors and ceiling of the walk in freezer.
      • The following non food contact surfaces were observed soiled and require cleaning: 1. heat lamps above the fry station 2. up splash of all the beverage equipment, including coffee makers in the drive thru area 3.the plastic containers used to store pastries 4. wire racks of the dish washing area holding clean equipment 5. up splash of the coffee brewers by the front service area
      • Observed the two mop sinks of the facility were missing a backflow preventer device or an air gap. During the inspection the hoses connected to the water faucets were hung up to avoid backflow contamination.
  • Tapa Rio, 1101 Sophia Street Fredericksburg, VA 22401
    • Date inspected: Dec. 14
    • Total violations: 2
      • No hand drying provisions were provided at the handwashing sink at the bar. Paper towel was provided during inspection.
      • Observed in-use serving scoop for potatoes stored in water at a temperature of 98’F.. Scoop was cleaned and placed in fresh water measuring 155’F. Person in charge said that they would monitor the temperature of the water.
  • Amy’s Cafe, 103 West Cambridge Street Fredericksburg, VA 22405
    • Date inspected: Dec. 15
    • Total violations: 4
      • The person in charge could not provide adequate parasite destruction documentation for salmon which may be ordered raw and/or undercooked. The person in charge provided documentation from fish supplier stating the fish is “parasite free” however, the documentation does not include the fish species or further information to evaluate how the fish are labeled parasite free, such as feeding practices. Instructed the person in charge to continue serving the salmon fully cooked until further documentation is obtained.
      • The person in charge could not provide adequate parasite destruction documentation for salmon which may be ordered raw and/or undercooked. The person in charge provided documentation from fish supplier stating the fish is “parasite free” however, the documentation does not include the fish species or further information to evaluate how the fish are labeled parasite free, such as feeding practices. Instructed the person in charge to continue serving the salmon fully cooked until further documentation is obtained.
      • Raw and/or undercooked eggs, steaks, salmon and hamburgers are offered on the menu, menu board, table tent or brochure without proper disclosure. Observed an asterisk located next to the general menu item description, which does not specify the animal derived food which may be served/ordered raw and/or undercooked. The person in charge reported menu revision is in progress and a copy of the proposed new menu will be emailed to the health department for review.
      • Observed improper methods to adequately cool French onion soup and portioned bags of pasta. Observed French onion soup stored in a deep and covered plastic container and the pasta was stored in densely packed portioned plastic bags. The person in charge reported an ice bath was used to cool the soup, however, the ice bath did not reach the top of the food container. Discussed ensuring the ice bath reaches the highest level of the food container during cooling. Strongly encouraged monitoring cooling temperatures.
  • Five Guys Burgers and Fries, 570 Celebrate Virginia #153 Fredericksburg, VA 22405
    • Date inspected: Dec. 15
    • Total violations: 7
      • The person in charge could not provide verification of employee health training. Food employees were knowledgeable of symptoms and illnesses requiring reporting. The general manager was contacted, and employee health training documents will be emailed.
      • Through discussion with the person in charge the milkshake machine spindle is washed and sanitized once daily; the spindle is sanitized between uses. Informed the person in charge the milkshake spindle is required to be cleaned and sanitized once every four hours.
      • Observed absence of beard restraints.
      • Observed the base of the cabinet under the beverage machine in poor repair The base is eroding and deteriorating in the back and there appears to be water damage. Also observed gap around the drain preventing effective cleaning. Ensure the base of the cabinet is in good repair.
      • Observed long hose with a spray nozzle (on a hose reel) attached to the mop sink with a Y-connecter in absence of a backflow preventer. The water was turned off and sprayer removed during the inspection. Instructed the person in charge to purchase a dual check valve with atmospheric vent and emailed a backflow preventer handout. Instructed the operator to discontinue use until the backflow preventer is installed.
      • Observed accumulation of food and other debris around the dumpster enclosure.
      • Observed the following in poor repair: Rust on air vent in the men’s toilet room, gap in wall with the conduit lines under the right side soda machine in the dining room and one ceiling tiles is not secured to the frame in the kitchen by the mop sink.
  • Little Caesars 3207-004, 5713 Plank Road Fredericksburg, VA 22407
    • Date inspected: Dec. 16
    • Total violations: 1
      • Ceiling throughout facility observed with residue and static dust and is in need of cleaning.
  • Staybridge Suites, 2996 Richmond Hwy Stafford, VA 22554
    • Date inspected: Dec. 19
    • Total violations: 7
      • Observed an employee walk through the kitchen to go to the breakfast area. EHS discussed this with the employee, the kitchen employee, and the hotel manager.
      • Observed that the facility does not have a certified food protection manager.
      • Per discussion with the kitchen person, he is continuing to wash his hands at the 3-compartment sink. EHS discussed this with the kitchen person.
      • Per discussion with the kitchen staff, the dish machine has not been repaired however he is continuing to use it to sanitize the dishes. The low-temp dish machine is not sanitizing the equipment. EHS discussed discontinuance of using the dish machine and setting up the 3-compartment sink.
      • Observed shredded cheese, in which time as a public health control is being used for, does not have a time annotated when it was taken out of temperature control.
      • Observed that the bulk cereals are not labeled
      • Observed several dead roaches inside of the storage cabinet in the kitchen.
  • El Molcajete Moya 2, 125 Corporate Drive Suite #105 Stafford, VA 22554
    • Date inspected: Dec. 19
    • Total violations: 2
      • Observed a large pot of Monteca that was made last night held at 78’F. EHS discussed cooling with the person in charge.
      • Observed a can of Lysol stored with the single service utensils. The can was removed.
  • Baba Ganoush Mediterranean Grill, 1115 Emancipation Highway Fredericksburg, VA 22401
    • Date inspected: Dec. 19
    • Total violations: 7
      • The facility does not have a person that is a Certified Food Protection Manager.
      • Observed a new installed (not approved by VDH) handwashing sink in the kitchen without paper towels. During the inspection paper towels were provided.
      • Observed a new installed (not approved by VDH) hands ink in the kitchen without any soap available.
      • Observed the facility does not have a date marking system in place for the food items being prepared in house.
      • The following non food contact surfaces remain in need to be cleaned: 1. The gas range-a lot of food debris was observed 2. The interior surfaces of the freezing and cooling units.
      • Observed the facility does not have a covered trash can or receptable in the women’s bathroom.
      • Observed in the shared hallway the facility is storing unused equipment.
  • Team Carolina BBQ, 18164 A P Hill Blvd Bowling Green, VA 22427

      • The 3 compartment sink needs to be sealed to the wall.
  • 25 30 Espresso II, 7501 Graham Street Suite 120 Spotsylvania, VA 22553

      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Observed mold-like substance on strawberries in small 3-door refrigerator. Strawberries discarded during inspection.
      • Observed Raid pest spray stored on shelf next to food containers at front prep area. Pest spray was relocated during inspection.
      • Shelving wall below front counter is constructed of raw, unfinished wood and is not smooth and easily cleanable.
  • Subway (Harrison Crossing), 5769 Plank Rd Fredericksburg, VA 22407
    • Date inspected: Dec. 19
    • Total violations: 7
      • The Person in Charge was unable to provide documentation of training relating to the Employee Health Policy for this facility Facility provided with Employee Health Policy that was reviewed and signed by all available employees.
      • Facility does not have written procedures for the clean-up of vomiting or diarrheal events. Facility provided procedures during inspection.
      • Observed food employee donning gloves without washing hands prior. Employee was informed to wash hands prior to donning gloves.
      • Sanitizer concentration in sanitizing basin of 3-compartment sink observed at 0 ppm. After observation, the sanitizer dispenser was not working properly. Person in Charge provided alternative sanitizer, which was measured at 100ppm during inspection. Food equipment in sanitizer basin was sanitized properly.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Observed pipe leaking under back handwashing sink.
      • Flooring throughout back area of facility was observed to be soiled and is in need of cleaning.
  • America’s Best Wings, 1917 Plank Road Fredericksburg, VA 22401
    • Date inspected: Dec. 20
    • Total violations: 12
      • During the inspection EHS observed an unauthorized food employee (an employee from the next door business) walk into the back of the kitchen.
      • Observed a food employee scrub soapy hands for less than 5 seconds and had not yet wet their hands with water first. EHS demonstrated to 3 food employees proper hand washing techniques.
      • Observed food debris (rice) in the hand sink of the kitchen. During the inspection a food employee cleaned out the sink.
      • Observed in the walk in cooler 9 cardboard boxes of cooked chicken wings that were stored in the same boxes that raw chicken were being received in. During the inspection the cooked chicken wings were discarded. EHS spoke with the person in charge to use plastic containers or other approved food contact surfaces to stored cooked chicken wings.
      • Through discussion with the person in charge the sauce bowls used to toss the wings after they have been cooked currently under go washing at the end of the day only. EHS spoke with the person in charge to increase the wash rinse and sanitize frequency of food contact surfaces (bowls) to every 4 hours.
      • Through discussion with the person in charge the burger and steak and cheese sub may be ordered undercooked. Observed the menu had no consumer advisory for these two products offered undercooked.
      • Observed two bottles of raid in the kitchen area. During the inspection the bottles of raid were removed. EHS spoke with the person in charge to contact pest control
      • Observed two fly strips in the kitchen area between the cooks line and walk in cooler.
      • Observed food employees working while wearing wrist jewelry.
      • Observed food employees working without hair restraints.
      • Observed several clear plastic food containers being stored stacked while still wet.
      • Observed the following non food contact surfaces in need of cleaning: 1. wire racks holding foods in the walk in cooler 2. non food contact surfaces of the prep food area (near microwave) 3. inside surfaces of reach in prep and under counter coolers 4. handles of all cooking and cooling equipment.
  • Country Inn & Suites, 5327 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Dec. 20
    • Total violations: 3
      • The Person in Charge was unable to provide documentation of training relating to the Employee Health Policy for this facility Facility provided with Employee Health Policy that was reviewed and signed by all available employees.
      • Facility does not have written procedures for the clean-up of vomiting or diarrheal events. Facility provided procedures during inspection.
      • Facility was unable to provide test strips to test the concentration of the sanitizer solution. Provided test strips to facility until they receive more.
  • Fas Mart #2, 2515 Salem Church Road Fredericksburg, VA 22407
    • Date inspected: Dec. 20
    • Total violations: 5
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Walk-in refrigerator door observed not to be closing properly to form a seal, equipment is in need of repair.
      • The following non-food contact equipment were observed to be soiled and are in need of cleaning: 1. Shelving in walk-in refrigerator; 2. Flooring in walk-in refrigerator; 3. Mop sink area; 4. Inside bottom of oven; 5. Exterior of fryers; 6. Cart next to fryers; 7. Shelving under front display hot box.
      • The flooring throughout the back storage and prep area and the flooring under the fryers were observed to be soiled and are in need of cleaning.
      • Observed unused equipment (microwave, hot box, pizza oven, prep unit) stored in back prep area of food facility and is in need of removal from premises.
